Ward I Dear Mayor Hurley and Council members
CAROLYN I would like to request approval for a change order to increase the contract amount by
COLE $10,200.00. This will make for a total net increase of $3,495 in the contract amount.
Ward 2 Iowa DOT has already approved the changes as shown on the enclosed Iowa DOT
HAROLD change order form. This is fully (100%) reimbursable.

Ward 3 The following is a list of the changes:
1 Additional 5 section programmable signal head $4,750.00
JOHN A. 2 Additional asphalt and rock because of larger than anticipated $2,200.00
KINCAID drainage area excavation
Ward
3 Additional conduit and cables due to the larger than anticipated $3,250.00
RON drainage area excavation (existing had to be terminated due to
WELPER excavation)
Ward 5 Total: rt10,200.00
BOB GREENWOOD Waterloo Cityhas already approved a $6,705.00 reduction in the contract
At-Large amount. As a result the net change in contract amount will be $3,495.
