You are hereby ordered to make the following changes from the plans and specifications or perform the
following extra work on your contract dated May 9, 2005
A. Description of change to be made or extra work to be done:
1) Change trench drain type to ABT trench drain.
2) Change some components of the trusses from that specified on the plans.
B. Reason for ordering change or extra work:
1) Requested by the contractor and agreed by the city's Engineering Dept.
2) Upgrading the truss design to current standards.
C. Settlement for cost of work to be made as follows:
Item no. 1: $- 8,055.00
Item no. 2: $+1,350.00
TOTAL: $ -6,705.00
