RE: Change order requests for F.Y,2005 US 218/west 11`"street traffic safety
improvements Contract#671
This letter is In request of a change order. There are two items that this change order is need for. The first is the trench drain
on 11`h street;Todd Van Dorn has requested that an ABT trench drain be used instead of the specified trench drain. If he is
avowed to make this change there would be a deduction of$8,055.00 to the contract. This deduction is do to the need for only
one utility access instead of three and bid item 34 trench drain gets decreased to$25,740.00 instead of$27,195.00. The new
price for the trench drain Includes the custom catch basin.
The second Item is the extra cost accrued to up date to the new Iowa Department of Transportation sign cantilever support
standards. This needed to be done since the design used in the plans was not up to date with the new standards. The cost
that was accrued to make this up date is$1,350.00
The net change order would deduct$6,705.00 from the contract.
Please let me know If this change order is approved so that materials can be ordered.
