LABOR:
1. Name of employee
2. Job title of employee
3. Dates employee worked
4.Number of hours worked each day, including regular time and overtime
5. Rate of hourly pay,both regular time and overtime
6. Fringe benefit rates for regular time and overtime, must give breakdown.
7. Copy of timesheets or time cards for each employee for claimed dates/times
APPLICANT OWNED EQUIPMENT
1. Description of equipment used(type,horse power, capacity, KW rating, etc)
2. Dates used
3. Number of hours used each day
4.Name of equipment operator(must be included in LABOR above)
5. Description of what the equipment was used for
RENTED EQUIPMENT
1. Copy of invoice from rental company
2. Copy of receipts for fuel used in rental equipment
3. Equipment description (size, capacity, horse power, make &model
CONTRACTS
1. Name and address of contractor
2. Dates worked
3. Copy of invoice
4. Copy of contract
5. Description of work done
MATERIALS
1. List of materials used(tools, equipment, supplies, foodstuffs, etc.)
2. Invoices or receipts for each item
3. Unit cost of each item
4. Dates purchased and used
5. Identify as "from stock" if used for storm response from inventory and not
purchased.
DOCUMENTS, IF APPLICABLE
1. Copy of Insurance Policy
2. Overtime Labor Policy
3. Emergency Plan
4. Mutual Aid Agreement
