All Bidders submitting a bid on the above contract shall carefully read this Addendum and give it
consideration in the preparation of their bid.

I. The following is a revision in the estimated quantity in the Form of Bid or Proposal:

II. On Plan Sheet 3, GENERAL NOTES, NOTE 5: Replace existing wording with the
following:
THE CONTRATOR SHALL FURNISH THE REQUIRED STAKES FOR THIS WORK.
CONTRACTOR SHALL MAINTAIN ALL STAKES AND REPORT ANY DAMAGE TO THE
ENGINEER. CONTRACTOR SHALL VERIFY ALL GRADES, LINE, LEVELS AND
DIMENSIONS AS SHOWN ON THE DRAWINGS, AND SHALL REPORT ANY ERRORS OR
INCONSISTENCIES TO THE ENGINEER PRIOR TO COMMENCING WORK. CONTRACTOR
SHALL BE RESPONSIBLE FOR PRESERVING SURVEY STAKES AND MARKS, AND IF ANY
SURVEY STAKES OR MARKS ARE CARELESSLY OR WILLFULLY DESTROYED OR
DISTURBED BY THE CONTRACTOR, HE SHALL BE RESPONSIBLE FOR THE MISTAKES
THAT MAY BE CAUSED BY THEIR LOSS OR DISTURBANCE AND SHALL BE
RESPONSIBLE FOR THE COST OF REPLACING THEM.
III. Use revised Form of Bid Pages 5 of 11 pages included in this addendum.
Any revisions to any of the Contract Documents made by this Addendum shall be considered as
the same revision to any and all related areas of the Contract Documents not specifically called out
in this Addendum.
The bidder shall acknowledge receipt of this Addendum by inserting the number and date in the
spaces provided;`in the Form£of Bid or Proposal, Item 9, Page 10 of 11.
