INSTRUCTIONS FOR PREPARATION AND MAILING ANNUAL REPORT OF UTILITIES
(This form is to be used for Electric Plants, Water Works, Gas Utilities, and Steam Heat Operations)
Due to an increased demand for more detailed statistics on municipally owned utilities, we are making up a
separate report form to be used by the city and town officials for reporting these transactions.
The accounts listed on the following pages are those prescribed in the accounting manual of the National
Association of Railroad and Utilities Commissioners, 5310 ICC Building„ P. O. Box 684, Washington 4, D. C.
These accounting manuals are available on request to the above address at a cost of$3.50 each.
These operating revenue accounts have been prescribed for municipal electric and gas utilities by the Iowa
State Commerce Commission in the Commission's Public Utilities General Order No. 3, dated December 24,
1963.
Statements call for reporting Cash Receipts and Disbursements, Summary of Cash Transactions, Report of
Indebtedness, Free Service Rendered, Reserves for Sinking Funds, and List of Officials.
If you desire to include a balance sheet and operating statement, we would be glad to have them included
with the report. Also, if an annual report is printed in pamphlet form, we would be glad to have that included.
Three copies of the report form are sent to you, and if the utilities are managed by a Board of Trustees, the
report is to be filled out by them and two copies filed with the city clerk or auditor, as provided by Section
368A.7, 1962 Code of Iowa.
A copy of this form shall also be used as the Annual Report to the Iowa Commerce Commission (Utilities
Division). Section 9 of Chapter 286, Code of 1962, and Commerce Commission General Order #3 prescribe
the accounting requirements for non-rate regulated utilities. It should be filed on or before April 1, 1966 with the
Iowa Commerce Commission in care of the Utilities Division, East 12th and Court, Des Moines, Iowa.
The Commerce Commission will use their copy of the report to verify the accuracy of the Gross Operating
Revenue Report which is the basis of their annual assessment to each municipal utility.
One copy of this report must be forwarded to the State Auditor's Office, State Capitol Building, Des Moines,
Iowa, not later than February 1, 1966.
You should retain one copy for your permanent files for future reference.
SPECIAL NOTE
The fund balance shown on line 1 of the Cash Transactions Schedules on pages 5, 7, 9, and 11 for December
31, 1964 must be the same amount as shown on Line 5 of the same schedule for your 1964 annual report. If any
adjustments were made to the ending balance in 1964 they must be shown in order to tie out with your begin-
ning fund balance as of January 1, 1965.
