GETTY Please find attached agreement for Professional Services with Earth Tech,Inc. of Waterloo,Iowa,
Ward 3 in conjunction with the above-referenced project. This is the first supplemental agreement to the
original professional services agreement dated April 3rd,2006.

KINCAID Services provided under this agreement include construction staking,review, materials testing,
Ward right-of-way staking, contract administration and record drawings. This agreement also includes
RON subconsultant services provided by Weston Solutions for management of contaminated
WELPER subsurface soil.

This is a cost plus fixed fee agreement for a maximum amount payable of $161,000.00. This is a
BOB Congressional designated project, and therefore,work under this agreement will be paid 100%
GREENWOOD from federal earmarked(Demonstration)funding provided under SAFETEA-LU(Safe,
At-Large Accountable, Flexible,Efficient Transportation Equity Act: A Legacy for Users)
