Recommended City Council Action:
It is recommended that this project be accepted.
Summary Statement
I report that HELMKAMP CONSTRUCTION OF IOWA, of Ankeny, Iowa, has completed
the F.Y. 2009 BIG ROCK ROAD BRIDGES REPLACEMENT WITH BOX CULVERTS,
CONTRACT NO. 739 in accordance with the plans and specifications. The total cost of the
contract was $248,259.19.
Transmitted also is the Maintenance Bond that guarantees the maintenance of this improvement
for a period of two (2) years.

Dear Mr. Gentz:
The above-referenced project has been completed and constructed by the contractor, Helmkamp
Construction of Iowa, in accordance with the plans and specifications completed by AECOM and
dated March 12, 2009. The final paperwork for the project, including the lien waivers, final
increase/decrease change order, maintenance bond and the final pay estimate to release the
contractor's retainage has been submitted previously. Construction record drawings will be
submitted later this week.
We appreciate the cooperation we have received from the city of Waterloo Engineering
Department throughout this project. Should you have any questions or require additional
