TO ALL BIDDERS ON THE ABOVE PROJECT
All Bidders submitting a bid on the above contract shall carefully read this Addendum and give it
consideration in the preparation of their bid,
I. The following are revisions to the FORM OF BID OR PROPOSAL:
1. The first page of the FORM OF BID OR PROPOSAL is revised so that when reading the
estimated quantity column, the estimated quantities listed are fully visible. A copy of the
first page only of the FORM OF BID OR PROPOSAL, marked Revised 4/2/09, is
attached and considered part of this addendum. There are no other revisions to the
remaining pages of the FORM OF BID OR PROPOSAL. When submitting your bid on
the loose Bid Packet form you received with your plan set, replace the first page only
with the attached revised first page. The remaining pages in the Bid Packet do not have
any revisions and should be used as originally intended in the Bid Packet when
submitting your bid.
II. Any revisions to any of the Contract Documents made by this Addendum shall be considered as
the same revision to any and all related areas of the Contract Documents not specifically called
out in this Addendum.
V. The Bidder shall acknowledge receipt of this Addendum by inserting the date and number in the
spaces provided in the revised FORM OF BID OR PROPOSAL, marked Revised 4/2/09.
