RESOLUTION APPROVING F.Y. 2015 CLAIMS SERVICE
CONTRACT AGREEMENT FOR THIRD PARTY
ADMINISTRATION WITH ALTERNATIVE SERVICE
CONCEPTS, LLC.
BE IT RESOLVED BY THE COUNCIL OF THE CITY OF WATERLOO, IOWA,
that the F.Y. 2015 Claims Service Contract agreement dated
June 9, 2014, for Third Party Administration at a cost not to
exceed $23,810.00, plus bill review, to provide administrative
and claims services for self-funded workers' compensation claims,
for the period of July 1, 2014 through June 30, 2015, be and the
same is hereby approved.
PASSED AND ADOPTED this 9th day of June, 2014.
