RESOLUTION NO. 2014-450
RESOLUTION APPROVING RENEWAL OF CONTRACT WITH
WELLMARK BLUE CROSS BLUE SHIELD AND DIRECTING
EXECUTION OF SAID CONTRACT BY MAYOR.
BE IT RESOLVED BY THE COUNCIL OF THE CITY OF WATERLOO, IOWA,
that the contract dated June 9, 2014, for the renewal of stop
loss health insurance for a premium of $915,245.00 per month, and
to administer the employee health care benefit plan and
prescription plan, including administrative and claim services at
a cost of $27.81 per employee per month and a disease management
addendum, for the period of July 1, 2014 through June 30, 2015,
by and between Wellmark Blue Cross Blue Shield and the City of
Waterloo, Iowa, be and the same is hereby approved, and the Mayor
authorized to execute the same in behalf of the City of Waterloo,
Iowa.
PASSED AND ADOPTED this 9th day of June, 2014.
