RESOLUTION NO. 2014-451
RESOLUTION APPROVING RENEWAL OF CONTRACT WITH
DELTA DENTAL OF IOWA AND DIRECTING EXECUTION
OF SAID CONTRACT BY MAYOR.
BE IT RESOLVED BY THE COUNCIL OF THE CITY OF WATERLOO, IOWA,
that the contract dated June 9, 2014, for the renewal to
administer the employee dental health care benefit plan,
including administrative and claims services at a cost of $3.91
per employee per month, for the period of July 1, 2014 through
June 30, 2015, by and between Delta Dental of Iowa and the City
of Waterloo, Iowa, be and the same is hereby approved, and the
Mayor authorized to execute the same in behalf of the City of
Waterloo, Iowa.
PASSED AND ADOPTED this 9th day of June, 2014.
