A final review of the above -referenced project was held, and the project is considered complete as of
November 18, 2016.
2. To the best of our knowledge and belief, based on observations of City personnel and AECOM staff
during construction, the contractor, Municipal Pipe Tool Company, LLC, has performed the work in
accordance with the plans, specifications and contract documents in effect for the above -referenced
project.
3. The total cost of the completed work is $1,734,590.95.

17.2016. Subcontractor hereby forever waives and releases any and all mechanic's lien, lowa
Code Chapter 573, and federal Mjller Act dainis, and any related rights and causes ofaetion in
respect to the Projeet and the property an which it is Iocated to the extent ofthe above payrnent
amount due and alt previous payments reeeived. Subcontraetor hereby forever waives and
releases any and alt claims, causes of action, and rights to payment he/she/it may have with
respeet to the Project, ineluding but not limited to any claims, eauses ofaetion. and rights under
or against the Iowa Mechanic's Lien Chapter (lowa Code §572. 1 et seq.), Iowa's Little Mitier
Act (Iowa C.ode §573. 1 et seq.), the federal Milkr Act (40 U.S.C. § 3131 et seq.), any retainage
hetd by the owner of the Project, and any surety bonds issued by Contractor's Surety for the
Project. Subeontractor also forever waives and releases any and all claims. causes of action, and
rights against the Projeet Owner and Contractor's Surety with respect to the Project to the extent
ofthe above payment amount due and alt previous payments received.
