Article III.C.2 CONSTRUCTION ADMINISTRATION &
Article III.C.4 CONSTRUCTION OBSERVATION &
Article III.D STORM WATER POLLUTION PREVENTION PLAN AND MONITORING
Additional construction related services for administration, observation, and SWPPP monitoring for
work performed by the contractor after the substantial completion deadline of December 31, 2015
and final completion deadline of March 31, 2016. Remaining work performed by the contractor after
the substantial completion deadline required nearly continuous observation services. These
additional services covers work from January 1, 2016 through the anticipated final completion of
May 13, 2016. Compensation for the additional fees are divided by each site per the following table:
Hollywood Lift Station

are additional services, scope of which is not contained within the original scope of services defined in the
original agreement. The Additional Services in this Change Order are subject to the general terms and conditions
contained in the Original Agreement dated July 18 2011 .
