HomeMy WebLinkAboutStanley Consultants - Supplemental Agmnt No. 1-7/1/2013SUPPLEMENTAL AGREEMENT NO. No. 1 This Supplemental Agreement, made and entered into by and between STANLEY CONSULTANTS, INC. (Consultant) and CITY OF WATERLOO, IOWA (Client) amends their agreement of April 25, 2011, for Consultant to provide services to the Client for the Storm Water Lift Station on Westfield Ave, Virden Creek and Vinton/18th Street Project, as follows: Scope of Services Additional Services requested by CLIENT and agreed to by CONSULTANT for groundwater dewatering monitoring, data logging and review of the data. Time of Beginning and Completion Beginning on June 25, 2013, for a period of approximately 90 days Fees and Payments Hourly Basis for Direct Labor, Plus Reimbursable Expenses in accordance with Hourly Fees and Charges Fiscal Year 2013-2014 (Form BC_C 13-14) copy attached. Estimated Hourly Fees and Reimbursable Expenses include: Setup and Data Collection 8-12 hours @ $126.00/hr Monitoring 2 hours/week @ $126.00/hr Equipment Rental at Cost Plus 10% @ $926/month x 10% = $1,019/month (Water Level Meter $100/month x 1 ea; Levelogger $220/month x 2 ea; Solinst Direct Read $83/month x 1 ea; Solinst Barologger $234/month x 1 ea; Solinst 25' Cable $32/mo; Solinst 50' Cable $37/mo) Except as specifically amended by this Supplemental Agreement, all the terms and conditions of the original Agreement dated April 25, 2011 shall remain in full force and effect. When traveling by public carrier, the maximum charge will be eight hours per day. II. Compensation for items of expense and other charges incurred in connection with the performance of the work shall be in accordance with the following schedule: Automobile $0.65/mile Automobile Assigned to Project Site $45.00/cal. day Four -Wheel Drive Vehicles $0.80/mile Four -Wheel Drive Vehicles Assigned to Project Site $55.00/cal. day Ground Transportation (rental car, taxi, etc.) At Cost PIus 10% Air Travel (commercial and charter) At Cost PIus 10% Living Expenses (away from assigned office) At Cost PIus 10% Telephone and Facsimile At Cost Plus 10% Global Positioning System Receivers $18.50/hour Equipment Rental At Cost PIus 10% Laboratory Work At Cost Plus 10% Soils Testing and Analysis At Cost Plus 10% Outside Photographic Work At Cost PIus 10% Duplicating Work (schedule supplied upon request) Technical Testing and Surveying Equipment (schedule supplied upon request) III. Compensation for purchases, items of expense, and other charges not scheduled above, incurred in connection with the performance of the work, shall be at cost plus 10%. IV. Interest at the rate of 11/2% per month will be charged on invoices not paid within 30 days. V. Charges are subject to revision on or after March 31, 2014. Stanley Consultants, Int.
